Ed Pilgrim, PT. DPT

Ed Pilgrim is the founder of STL Physical Therapy and a performance physical therapist committed to bridging the gap between rehabilitation and athletic performance. He earned his Doctorate of Physical Therapy from the University of Missouri while simultaneously becoming a Certified Strength & Conditioning Specialist, building a strong foundation in movement assessment and sport-specific training.

After graduating, Ed worked with a wide range of clients — from the general population to elite athletes in the MLB, NFL, UFC, and the SEC. In 2023, he returned home to bring the same level of treatment and performance coaching typically reserved for professional athletes to the city of St. Louis.

In March of 2025, Ed founded STL Physical Therapy after recognizing that traditional, insurance-based clinics weren’t designed to meet the needs of active individuals and youth athletes. Rushed appointments, symptom-focused treatment, and insurance limitations often prevent athletes from receiving true one-on-one care while addressing the root cause of their injuries. Ed created STL Physical Therapy to deliver individualized, high-quality, 1-on-1 care that restores movement, enhances performance, and returns athletes to better than they were before.

Josh Patterson, PT, DPT, CMPT

Josh is a performance physical therapist at STL Physical Therapy in Creve Coeur, Missouri, where he helps active individuals and athletes return to sport with confidence and long-term durability. He earned his Doctorate of Physical Therapy from Washington University in St. Louis, where he built a strong foundation in movement analysis and evidence-based treatment.

Josh is a Certified Manual Physical Therapist (CMPT) and is certified in dry needling, allowing him to blend advanced manual therapy with targeted strength and performance strategies. He specializes in treating lower extremity injuries, with a particular passion for working with jumping and cutting athletes such as those in basketball, volleyball, and track and field.

A former basketball player and lifelong fan of the game, Josh especially enjoys working with basketball athletes, combining clinical expertise with personal experience to help them not just recover, but perform at a higher level.

Tyler Forbes, PT, DPT

Tyler Forbes is a Doctor of Physical Therapy with over a decade of experience in orthopedic and sports rehabilitation. He earned both his Doctorate of Physical Therapy and Bachelor of Health Science degrees from Maryville University, where he also played collegiate soccer. His clinical focus includes sports performance, return-to-sport testing, concussion and vestibular rehabilitation, and injury prevention across all levels of athletes — from youth organizations to professional athletes in basketball, soccer, MLB, and the NFL.

Dedicated to advancing the profession, Tyler has served as an Adjunct Professor in Maryville University’s Doctor of Physical Therapy program, teaching courses in modalities, clinical decision-making, and advanced orthopedic assessment. He is also an active mentor for new graduates and emerging leaders, helping to cultivate the next generation of clinicians and managers. Tyler has contributed to multiple research studies presented at national and state physical therapy conferences, including work on ACL injury risk in adolescent athletes and biomechanics in collegiate soccer players.

When he’s not in the clinic, Tyler enjoys working with youth sports organizations, coaching youth sports, and spending time with his wife and two kids in the Ozarks. He especially loves working with soccer athletes and has extensive experience guiding players through concussion rehab and return-to-play progressions safely.
